The Department of Care Management utilizes the Team Leader model to enhance the operational and clinical effectiveness of the department and to ensure seamless management of the day to day activities of staff engaged in discharge planning, care coordination activities, social work and capacity management.
In addition to the key responsibilities associated with the roles and scope of Nurse Care Manager or Care Manager-Social Worker, the team leader key responsibilities include:
Serves as clinical expert for staff within a designated area and acts as first level consultant for staff within designated team to address patient care issues, discharge barriers, etc.
Advises staff appropriately and includes departmental leadership as necessary.
Serves as point of contact and mentor for members of the designated team.
Monitors census daily to ensure appropriate staffing and census distribution within the designated team.
Schedules and conducts team meetings/huddles (weekly)
Manages PTO and holiday staffing for designated team.
Improves and sustains patient flow, length of stay and capacity maangement
Communicate and reports activities of designated team members to departmental leadership
Weekends position: Friday/Satruday/Sunday
