Position Summary

The Nurse Navigator provides individualized assistance to patients and their families having procedures in the incumbents respective department by overcoming barriers to care, facilitating timely scheduling of procedures, and providing education and support post procedure. The patients' trajectory through the scheduling procedure, day of procedure education, and post procedure care is coordinated in collaboration with the multidisciplinary team. Additional patient navigational needs may be assigned to this role.

Minimum Requirements

Associates degree or higher in Nursing required. Bachelors Degree preferred. (RN - Registered Nurse)

3-5 years nursing experience required.

Knowledge, Skills & Abilities required:  (i.e. supervision, computers, etc.)  

Demonstrated ability to manage, teach, make independent decisions and assume responsibility

Strong interpersonal communication skills, verbal and written

Ability to communicate and work effectively with all levels of staff 

Working knowledge and experience with PC-based applications, i.e. Word, Excel, PowerPoint, Outlook

Ability to function as a leader, team member and execute the desired outcomes

Must exhibit creative, positive problem-solving abilities

Ability to foster collaboration between multiple parties

Ability to think conceptually and apply concepts in a practical application

Ability to assimilate information from a variety of sources, analyze information, and determines a course of action

Ability to consider diverse opinions and incorporate them into the decision-making process 

Commitment to a high standard of customer service

This is a full-time position - 40 hours per week - Monday - Friday.
