Position Summary
- The Patient Flow Expeditor plays a pivotal role in improving patient access/flow and enhancing quality of care using clinical expertise, leadership, organizational awareness, and critical thinking skills.
- The Expeditor's focus is the middle flow (i.e. between admission and actual discharge) of the care progression spectrum focusing on resolving any delays in care and/or discharge barriers. This person will focus on actions that are patient specific to improve patient experience and flow. Additionally, the Expeditor will work closely with Operations leadership to prioritize work for the day
- The Expeditor is expected to utilize real-time and predictive information; he or she will monitor, triage, and address alerts and delays (ex: patient itineraries, care progression barriers).
- The position will investigate throughput barriers and delay using the electronic medical record and/or contacting the appropriate parties and determine the appropriate action for resolution. Once a resolution path is decided, the Expeditor will communicate / work in partnership with the local care team to support resolution.
Responsibilities
- In depth understanding of the alerts, trends, flags and information provided on the tiles; able to identify patterns requiring proactive strategies to assure patient progression.
- Triages alerts via investigation with the electronic medical record, conversations with the care areas, and other available resources.
- Triages barriers to discharge to optimize impact on hospital capacity and free up high demand beds.
- In real time, expedites, prioritizes, synchronizes and mediates clinical issues, removes barriers to discharge and addresses delays in care.
- Liaises with Operations team leadership, team members, and providers regarding clinical priorities and synchronization based on system needs.
- Collaborates with operations center and acute care leadership to identify process improvement strategies, reduce patient length of stay (LOS), trend delays, support quality initiatives and enhance patient safety.
- Participates in department and organizational performance improvement activities.
- Manages deviations from patient care pathways; resolves bottlenecks/variation in service level agreements, prioritizes with support services, and communicates with a variety of clinical departments.
Minimum Requirements
- A bachelor's of science in nursing is required; master's degree preferred.
- 5 years of acute care nursing required.
- 2 years of Case Management experience preferred.
- Knowledge and experience using EPIC electronic health record.
- Licensure to practice as an RN in the state of NC.
