ECU Health Medical Center, one of four academic medical centers in North Carolina, is the 974-bed flagship hospital for ECU Health and serves as the primary teaching hospital for The Brody School of Medicine at East Carolina University. Position Summary

Pharmacists at ECU Health are committed to the safe and effective use of medications through maximizing drug distribution and optimizing drug therapy outcomes; educating patients, providers, and ourselves on best practices; and fully integrating pharmacists into the continuum of care. Pharmacists report directly to a designated Pharmacy Supervisor based on area of practice.

Responsibilities

Patient-specific medication management:

a. Reviews the patient's medication profile prior to verifying each new order. Evaluates orders for indication, dose, dosage form, regimen, route, therapeutic duplication and drug interactions.

b. Ensures appropriateness of therapy by: Reviewing patient medication history, assessing patient demographics, recommending formulary alternatives when appropriate, developing drug monitoring plans, interpreting laboratory data, monitoring and documenting pharmacokinetics for targeted drugs, which may include credentialing as a Clinical Pharmacist Practitioner.

c. Demonstrates basic competency in applying drug dosing guidelines, clinical lab data and pharmacokinetic properties to individualize drug therapy.

d. Communicates any drug therapy problems directly with the nurse and/or prescriber and documents all drug therapy interventions as appropriate.

e. Demonstrates the knowledge and skills necessary to provide care appropriate to the age(s) of the patients served.

f. Performs medication histories, medication reconciliation and discharge counseling as required.

g. Provides drug information to health care professionals and to patients.

h. Detects, monitors, documents and reports adverse drug reactions and medication errors.
Maintains good work relationships and handles all communications in a professional manner as reflected in peer and/or provider evaluations, as well as student and resident evaluations.
Supervises Pharmacy Technicians in daily work activities and collaborates with Supervisor as necessary to ensure excellence and professionalism.
Accurately dispenses investigational drugs according to established protocols and procedures.
Attends department education activities such as Grand Rounds/Noon Conferences, staff meetings, staff development programs, The Joint Commission (TJC) meetings, etc.
Completion required:

a. Obtains Continuing Education credits as required by the North Carolina Board of Pharmacy
b. Successfully completes all annual department competency assessments.
c. Completes the TJC annual requirements and occupational health screening.
d. Complies with all state and federal laws and regulations governing the practice of pharmacy.
Follows hospital and departmental policy and procedures.
Maintains confidentiality of medical records and complies with all applicable state and federal regulations, including, but not limited to, HIPAA and HITECH

Fulfills maintenance requirements for Clinical Ladder Level I.

Minimum Requirements

B.S. or PharmD in pharmacy

0 years of relevant pharmacy experience (post-graduate)

Must be licensed as a pharmacist by the NC Board of Pharmacy by start date.

Maintains active NC pharmacy license and provides proof of renewal prior to December 31 annually.

American Heart Association BLS is required within 30 days of hire.
