Position Summary

The Department of Care Management (CM) utilizes the Discharge Navigator role to enhance the operational and clinical effectiveness of the organization and department and to ensure seamless management of the day to day activities of staff engaged in discharge planning, care coordination activities, social work and capacity management.

Responsibilities

  • Serves as clinical expert for staff within a designated area and acts as first level consultant for staff to address patient care placement issues, discharge barriers, etc.  Advises staff appropriately and includes departmental leadership as necessary.
  • Serves as point of contact and expert resource for members of the team for processes related to coordination and authorization of post-acute services; SNFs, ALFs, post-discharge medication, discharge ambulance transfers, hospital to hospital transfers, LTACHs, etc. 
  • Monitors census daily to ensure appropriate, timely coordination of post-acute services and aligns interventions based on expected disharge dates and capacity needs. 
  • Improves and sustains patient flow, length of stay and capacity maangement through facilitation and coordination of post-acute services and placements. 
  • Communicates and reports activities of designated team members to departmental leadership as well as barriers and trends associated with our post-acute providers. 
  • Submits requests for authorization to payers for post-acute services and performs timely follow-up.

Minimum Requirements

  • Bachelor of Social Work degree from an accredited school of social work OR an Associate Degree in nursing from an accredited college is required.
