Position Summary
The wellness navigator works to engage and activate patients, team members and community in Lifestyle Medicine Clinic and Wellness services. Uses data and motivational interviewing skills to provide population and individualized assistance to clients to assess health needs, interests and goals and engage in wellness and clinical services. Answers inbound calls for well-being support line and lifestyle clinic referrals and makes outbound communications to activate patients and customers in services. The navigator coordinates referrals and patient scheduling for Lifestyle Medicine Clinic providers and programs. Assists clients to overcome barriers to engagement and address social determinants of health, with the objective of activating each client in the desired health behavior. Provides health education and assists with lifestyle and wellness program facilitation.
