Position Summary

The Radiology nurse navigator provides individualized assistance to patients and their families having procedures in the Radiology department by overcoming barriers to care, facilitating timely scheduling of procedures, and providing education and support post procedure. The patients' trajectory through the scheduling procedure, day of procedure education, and post procedure care is coordinated in collaboration with the multidisciplinary team. Additional patient navigational needs may be assigned to this Navigator.

Responsibilities

  • Communicates effectively with patients/families and other members of the health care team
  • Analyzes clinical situations in a systematic way and acts to obtain resources or correct problems to best meet the patient's health care needs.
  • Partners with unit leadership on quality and/or research initiatives to improve patient outcomes.
  • Work Collaboratively with referring providers to schedule timely radiology procedures. Discusses individual patient needs with providers to ensure requested procedure is appropriate and pre-procedure requirements are met.
  • Professional development model recognizes health care professionals who demonstrate advanced knowledge, critical thinking skills in their professional practice.
  • Create and maintain open lines of communication with physicians and their support staff.
  • Maintain a network with referring providers and support best practices in care delivery.

Minimum Requirements

  • Associates degree in Nursing required.
  • Bachelors Degree preferred.
  • 5 - 7 years nursing experience required
  • Demonstrated ability to manage, teach, make independent decisions and assume responsibility
  • Strong interpersonal communication skills, verbal and written
  • Ability to communicate and work effectively with all levels of staff
