Position Summary
Assist in providing direct quality patient care under the direction and supervision of a registered nurse. Participates in the assessment of patients. Assists in developing, implementing and evaluating/revising the plan of care. Attends educational programs and staff orientation which promote and professional development. Develops leadership role within the LPN scope of practice. Participates in Service Excellence Program.
Responsibilities
1. Assist in providing direct quality patient care under the direction and supervision of a registered nurse.
a. Assist in the coordination of intradepartmental and interdepartmental patient care activities and seeks to promote harmonious professional relations.
b. Utilizes nursing education to provide direct patient care and improve patient's physiological and emotional well-being.
c. Collaborates with members of the health care team.
d. Monitors and communicates changes in the patient's condition.
e. May document care in patient's medical record, procedures and nursing standards.
f. Acts as a role model for students and other employees.
2. Participates in the assessment of patients.
a. Collects and documents data about patients.
b. Assists in completing the nursing admission record consistent with policies and interviews patient/significant others to complete health history.
c. Documents care and patient's response to care within the framework of the standards of Nursing Service.
3. Assists in developing, implementing and evaluating and revising the plan of care.
a. Participates in implementing the plan of are through effective use of procedural and organizational skills.
b. Observes the patient's condition throughout the shift and informs the registered nurse of the patient's status.
c. Provides and promotes quality care in accordance with hospital policies and procedures utilizing nursing care standards.
4. Education
a. Participates in in-service education, staff meetings, committee meetings and continuing education to achieve quality patient care.
b. Fulfills annual mandatory education requirements.
5. Develops leadership role within the LPN scope of practice.
a. Follows hospital policies and procedures and unit protocols.
b. Reports changing patient status that affects unit staffing needs to charge nurse.
c. Adapts to changing patient care and unit situations.
d. Attends unit staff meeting unless excused.
e. Directs unlicensed personnel in the performance of tasks as delegated by supervising RN.
f. Assumes accountability both for responsibilities accepted and for any specific tasks which the LPN may direct others to perform.
6. Demonstrates appropriate, caring behavior in the workplace.
a. Treating patients, their families, visitors and staff with respect and dignity.
Minimum Requirements
Basic Life Support (BLS) Certification
Licensed Practical Nurse
