Position Summary
The Clinical Services Liaison (RN) is responsible for:
Positive and effective representation of The Center and introducing the full scope of services offered to hospital case managers, physicians, nursing agencies and other referral sources.
Ensuring the ongoing updates and education for referral sources.
Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of The Center.
Provides and utilizes clinical expertise and skills to assist referral sources &/or patients with:
the identification of appropriate candidates for home infusion &/or home health care.
the identification of the patients' and/or caregivers ability to provide care.
patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home.
medication connection at the hospital / facility in order to expedite the discharge into the home setting, as appropriate.
Minimum Requirements
Associate Degree in Nursing (ADN) or higher
Three years of relevant experience
Valid NC Nursing License
Valid NC Driver's License and auto insurance
BLS required within 30 days of hire; Must maintain current CPR certification
Other Information
This position is PRN/as needed - no benefits
Day shift - 8:30am-5:00pm, as needed to cover PTO or schedule request from full time CSL position.
