The Care Coordinator is responsible for the delivery of social work services and discharge planning to assigned BH patients. They provide direct interventions with patients and families jointly with the interdisciplinary treatment team. This includes assessment, treatment planning, individual and group psychotherapy, problem solving,safety, advocacy, community referrals and crisis intervention. It also involves management of discharge planning and treatment team meetings.
MSW or Master Level Allied Health discipline may be considered based on previous experience.
Licensure: Minimum provisional license required, full license (LCSW, LMFT, or LCMHC) preferred.
