ECU Health Roanoke-Chowan Hospital is a 114-bed community hospital located in Ahoskie. Serving four nearby counties, the 15-bed emergency department sees approximately 18,000 patients per year. ECU Health Roanoke-Chowan Hospital provides a wide range of health services, including medical, surgical and radiation oncology. In addition, the Wound Healing and Pain Management clinics serve as referral centers for neighboring ECU Health hospitals and providers.
The Case Management Discharge Coordinator (CMDC) position will serve to support the discharge planning process by communicating with and coordinating post discharge services and providers with oversight by the Nurse Case Managers (NCM) and/or the Social Work Case Managers (SWCM) within the Case Management Department. The CMDC may assist in collection of patient information as delegated by the NCM and SWCM, as well as work to facilitate a transition plan that promotes a safe and effective discharge. The CMDC will also perform other activities related to the discharge planning process as requested.
Associate Degree in Human Services related field required
Two years of healthcare related experience is preferred.
Experience in case management, doctor's office, home health agency, DME agency, or health care field is strongly preferred.
Knowledge of medical terminology and basic computer skills are required. Intermediate MS Word and Excel skills preferred.
Shift: Week Days - 8 hours
